PERSONALIZED CASE MANAGEMENT
A Plan Built Around the Person
There isn't one path out of homelessness. DIMH's case management approach begins by understanding each guest's individual circumstances, needs, strengths, and goals. Our case managers work alongside guests to identify barriers, connect them with resources, establish goals, and create practical steps toward greater stability.

Shelter is the beginning-not the destination. DIMH works with guests as they prepare for permanent housing, helping them navigate barriers and connect with resources for a successful transition.
90-DAY FOLLOW-UP PROGRAM
Support Doesn't End at the Door
Moving into housing is a major milestone-but maintaining stability after shelter matters just as much. Through DIMH's 90-Day Follow-Up Program, support continues after guests transition from shelter. Helping individuals and families navigate challenges, stay connected to resources, and build on the progress they've made.
